Gynecor's™ clients are provided with requisitions that are pre-printed with the practice contact information.

When you return the requisition, please fold it so that the patient information faces in, per HIPAA compliance. Simply attach the front and back copy of the patient’s insurance card.

It is important to complete the specimen grossing section. Attach the bar-coded stickers (provided at the bottom of the page) vertically to each vial. Mislabeled vials can delay your test results, as can incomplete requisitions.

Before you return the test back to Gynecor™, please provide the following: 

• CD-9 code
• Collection date
• Doctor’s name
• What test was performed
• Specimen type
• Vial count
• Patient’s sex (we are not allowed to assume that it is female)
• Insurance Information
• Practice name and address
